How to Enroll

You may use any of the methods below to enroll. During the enrollment process, you will need to have your Medicare card (and Medicaid card, if applicable) available to complete the application. If you need help or assistance while enrolling, please contact us.

Online

You may enroll online any time 24 hours a day. More importantly, online enrollment is safe and secure. If you have a legal representative, he or she may complete your online enrollment on your behalf. Clicking Enroll Online below will take you to the enrollment eligibility page. Upon reviewing the requirements on that page and selecting the checkbox which stands as your digital signature, you can begin enrolling online.

Medicare beneficiaries may enroll in Arkansas Community Care through the Centers for Medicare & Medicaid Services Online Enrollment Center, located at http://www.medicare.gov Link out. By Mail

Printable enrollment forms are available on this website. Select a plan in the left-hand column to view your plan information page which includes a link to open the .pdf form.

After downloading and printing the individual enrollment form, mail or fax the completed form to your local office. Our address and fax numbers are located on the contact us page.

For fastest processing time, please include a photocopy of your Medicare card (and Medicaid card, if applicable) with your signed enrollment form. Otherwise, you may receive a phone call from us in order to confirm your eligibility to enroll in our Medicare plans.

In Person

If you would like an information packet sent to you or to make an appointment with a plan representative, please contact your local office or email us.

You can also attend a free, no obligation informational meeting. Click here for a current list of informational meetings and to RSVP by email. You can also contact us to RSVP for the meeting and to get more information. A sales representative will be present at the meetings with information and individual enrollment forms. For accommodation of persons with special needs at sales meetings, please contact your local office.

What Happens after Enrollment?

After we receive your completed and signed individual enrollment form or online application, we will send it to the Centers for Medicare & Medicaid services (CMS) for approval.

Acknowledgement of Receipt of Application

Within 10 business days, you will receive a letter by postal mail acknowledging receipt of your completed enrollment form. This "Acknowledgement of Receipt Enrollment" letter can be used as proof of health insurance coverage before you receive your ID card in the mail.

Official Insurance Coverage ID Card and Welcome Kit

You will receive your new ID card within 10 calendar days after we receive your completed enrollment form. It is important to carry this card with you when you use your benefits. You will also receive a Welcome Kit that will include your printed Evidence of Coverage which explains all of your plan benefits and how to use your benefits.

Incomplete Applications

Within 3 business days of receipt of your application. we will contact you by postal mail if we require additional information to complete your application. This information could include verification of your eligibility based on your Medicare and/or Medicaid card number and your dates of entitlement to Medicare Part A and pre-enrollment in Part B. If you do not submit this information to us by mail or fax along with your enrollment form, we will call you to complete your application by phone.

Note: If you completed your application but used an invalid "Effective Date of Coverage" we will contact you and explain what effective dates you are eligible for. If you do not wish to begin your coverage on an officially-valid effective date, you may cancel your enrollment request. Please contact us for more information.

Ineligible Enrollments

If it is determined that you are not eligible for the health plan, a letter will be mailed to you within 10 calendar days.
